↵¶ We used Mondor et al.20 to define a history of chronic conditions. Eligible conditions included AMI, osteoarthritis, rheumatoid arthritis, asthma, cancer, cardiac arrhythmia, heart failure, chronic obstructive pulmonary disease, coronary syndrome (excluding AMI), dementia, diabetes, hypertension, mood, anxiety, depression and other nonpsychotic disorders, other mental illnesses, osteoporosis, renal failure and stroke (excluding transient ischemic attack).
